Application form

Wholesale buyers apply on your storefront with a form you design in Termpact. The store owner and admins can change it; everyone else can see it.

Open Customers → New customer requests → Sign-up form.

Every form has the fields Shopify needs to set up a B2B company:

Field Required Copied to Shopify as
Business name Always Company name
Tax ID / VAT / EIN Your choice Location tax registration ID
Website Your choice Company website
Shipping address Always Location shipping address
Billing address (if different) Your choice Location billing address
First name, Last name, Email Always Company contact
Phone Your choice Contact phone

You can rename standard fields, translate them and move them, but not remove them or change their type.

Choose Add field, then pick a type: short or long text, email, phone, website, number, address, country, dropdown, several choices, checkbox or file upload.

  • Labels and help text can be written for every language your store has published. Buyers see the form in their storefront language and fall back to your store’s main language.
  • Choices for dropdowns go one per line.
  • Limits: text can have a longest answer and a pattern it must match; numbers a smallest and largest value.
  • File uploads accept PDF, JPG, PNG or HEIC, up to 10 MB each and up to 5 files per answer. Tick This is a tax-exemption certificate to store the file with the location’s tax exemption once you approve the application.
  • A form holds up to 40 fields.

Set Show this field to Only when… and choose an earlier answer and a condition, for example: show Resale certificate only when Do you have a resale certificate? is ticked. Hidden fields are never required, and anything typed into a field that’s hidden when the buyer sends the form is thrown away.

Preview shows the form as applicants will see it on your storefront, in any language your store publishes. Conditional fields appear and disappear as you answer. You can’t send it from the preview.

Drag fields by their handle, or focus the handle and press Alt+Up or Alt+Down.

  • Save draft keeps your changes without showing them to buyers.
  • Publish makes your changes live straight away as a new version.
  • Discard changes throws the draft away and goes back to the live version.

Every published version is kept. An application always shows with the version it was sent on, so changing or removing a field later never loses what a buyer told you. Published versions lists each version and how many applications it received.

  • Message after sending: what buyers see after they apply, per language. Leave it empty for the default.
  • Countries that can apply: two-letter country codes, for example US, CA. Leave it empty to accept every country.
  • Email the store address about new applications: staff can also turn on their own notifications.

Applicants can reach the form in two ways:

  • Your own page: in the theme editor, open a page (for example a new “Wholesale” page), choose Add block → Apps → Wholesale application (Termpact) and save. The block uses your theme’s fonts, colours and buttons.
  • The ready-made page at /apps/termpact/apply on your store, which you can link from a menu. It opens inside your theme’s header and footer.

Customers who are logged in have their name and email filled in. Buyers who already have wholesale access see a link to their account instead of the form.

Sending the form needs JavaScript, which every current browser has on by default; it’s used to check that the applicant isn’t a robot and to upload files. Wholesale applications must be turned on for your store, and you need to have published the form; otherwise visitors see “Wholesale applications aren’t open right now.”
